The care concept of Primary Care (PC) or Primary Health Care (PHC) describes a health-oriented and multiprofessional care process close to the patient’s home. According to one of its common definitions in the WHO Declaration of Alma-Ata, PC/PHC fosters participation, self-determination, and development of personal and social skills. 

Based on the resolution of the Austrian Bundeszielsteuerungskommission (B-ZK) and the new Primary Care Act (PrimVG), such a model is currently being implemented to serve as a first point of contact for the Austrian population. At least 75 primary care institutions, so-called Primärversorgungseinrichtungen (PVE), are to be built by 2021. In Lower Austria, 14 such institutions are to be established. 

According to a base paper by the Lower Austrian Health Insurance Fund (NÖ Gebietskrankenkasse) and the Lower Austrian Medical Chamber (Ärztekammer NÖ), at least three more professional groups from the so-called extended team will complement the core team. Both of the two institutions established so far made their choice among the possible professions and decided for Social Workers to join their team. The third planned institution will also decide to involve Social Workers. 

This project aims to examine two different fields of study more closely. The first field of study is the structural integration of Social Work and the concrete Social Work practice in Primary Care and General Medicine as a field of action. The second field of study is dedicated to the existing classification and documentation systems of primary care with regard to their relevance and their relation to the procedures of social diagnostics and evaluation.
