People who repeatedly (e.g., within several hours or days) and despite interventions turn to family doctors, ambulances, hospitals, or primary care centres are referred to as “frequent flyers” within a clinical setting or “heavy users” outside the clinical setting. Next to underlying structural problems, such mislabelling often masks personal hardships such as homelessness, loneliness, psychiatric problem situations, or overstress in the fields of care and nursing. 

Identifying Alternative, More Suitable Resources

This project draws on the assumption that Social Workers are used at family doctors, emergency services, hospitals or primary care centres to identify alternative, more suitable resources for those affected and implement case plans together. 
•    What could case management look like in this context?
•    When / from which stage should Social Workers become involved?
•    Which methods, tools, and action-oriented concepts of Social Work are most applicable – and how should they be implemented?

The findings of this project will contribute to building a practical framework for Social Workers operating in health care settings.
